2. The thoracic CT shows variably-sized, circumscribed, non-calcified nodules bilaterally

The thoracic CT shows multiple, bilateral non-calcified circumscribed pulmonary nodules. The nodules are all at least 5-6 mm in size; some range closer to 2 cm. This size range is inconsistent with miliary disease- miliary nodules usually measure 1-2 mm in size. None of the nodules exhibit an association with enlarged vessels, and therefore the morphology of these nodules is not suggestive of arteriovenous malformation. Note that vessels are seen in contact with some of the nodules (see the anterior and medial left lower lobe nodule in Figure 2E), but these vessels are not abnormally enlarged and therefore the morphological appearance is not suggestive of arteriovenous malformation. The nodules are largely circumscribed (well-defined), and no ground-glass opacity is seen surrounding the nodules. None of the nodules show internal lucency- i.e., they are not cavitary.

Regarding the assessment of the thoracic CT findings, which of the following is correct?

  1. The nodules are highly suggestive of an aggressive malignancy
  2. The nodules are most consistent with a benign, post-infectious process
  3. The nodules are most consistent with a benign, non-infectious inflammatory process
  4. The nodules are indeterminate as regards their etiology and behavior, and any of the foregoing choices could be true
